At Centacare, we’re excited about the new possibilities the National Disability Insurance Scheme (NDIS) will bring for people with a disability. The scheme has been designed to maximise choice and control, and put you in the driver’s seat in determining the services and activities that can best help you achieve your goals.  So if you are living with a disability, this is an exciting opportunity to live the life you want, with a plan that meets your individual goals    

Am I eligible?

The NDIS provides people with a disability access to services to help them take part in everyday activities and to achieve their individual goals and aspirations.

The National Disability Insurance Agency (NDIA), works out who can access the scheme. You may be eligible if you are:

  • under 65 years of age when you first access the scheme
  • are an Australian Citizen or permanent resident
  • have a permanent or significant disability.


Centacare and the NDIS

As a registered provider of NDIS services Centacare is here to help you become NDIS ready. Our dedicated NDIS team is available to answer your questions. Once you get your plan, we will work with you to put it into action with the support and services you need to achieve your goals.


Your path to an NDIS plan

There are four key stages that need to be completed in your NDIS journey and Centacare is here to support you during this process.


Access the NDIS

The NDIA may have already contacted you about your eligibility, but if they haven’t you can contact the NDIA on 1800 800 110 to find out if you meet the access criteria and to begin the registration process.

The NDIA may post an Access Request Form to you to confirm your eligibility for the scheme (NDIS).

You may need to ask your health or education professional to complete Part F of the Access Request Form. This describes how your disability impacts your life.


Being prepared is the key to most things in life and the NDIS is no exception. That’s why Centacare is here to help you with the planning process. We have developed a handy pre-planning booklet that helps to highlight your needs and goals and most importantly, what support is needed.

NDIS planning meeting

If you’re eligible, the NDIA will get in touch with you to arrange a planning meeting. In the meeting they will work out the type of support and the amount of funding you need. You can invite others to this meeting.

Your first plan

Once you receive your first NDIS plan, it is time to put it into action and organise the support and services you need and Centacare can help with this.

If you are funded for Support Coordination, Centacare Choices can assist you to implement your plan with the help of one of our Support Coordinators.


Centacare services and support

We offer all the support you’d expect, but you won’t find a fixed menu of disability service and activity options. Our clients and their families know what’s best for them – it’s our job to listen, understand and together, come up with the service options, learning experiences and real life support that’s right for you.

We have services to help you live well at home, including support with your everyday personal routines, household chores and getting out for shopping and other appointments.

We can help you get the right living arrangements in place, with our supported group housing, short-term accommodation options or help to make the modifications you need to live better in your own home.
