There’s nowhere like home to warm the heart and soul. So why should you leave it?

Government-funded home care services can make a big difference when it comes to staying in the home you love for as long as possible. It might be a weekly fitness class to improve your strength and mobility, a trusted tradie to install a ramp so you can safely access your front garden or a regular card game to stay connected with friends. There are lots of options for support that will help you stay in your own home longer.

Centacare is an approved provider of services under the following funding schemes.

Home care funding options

Funding scheme What services are included? Who can access this funding?
Home Assist Secure provides access to home maintenance and modification services to help older Queenslanders live well in their own homes. Services include free information and referrals to trusted tradespeople, in-home assessments, minor modifications such as hand rails, ramps, smoke alarms, repairs and maintenance. To be eligible for Home Assist Secure services, you must be at least 60 years old (or any age with a disability), hold a Pensioner Concession Card, be unable to complete the work yourself and unable to have the work completed through alternative assistance (such as through family or friends or other funding schemes). Queensland Health Dementia Respite (QHDR) funding assists carers to maintain a healthy balance between their caring role and their work commitments. It also assists people who are living with dementia or a neurodegenerative condition to connect with their community. Services can include group social outings in the community, centre-based activities, assisted transport and at-home support. Carers of people who are living with dementia or a neurodegenerative condition, who are working or looking for work, studying or volunteering. Carers do not have to live with the person they are caring for to be eligible. Give us a call to find out more.
The Commonwealth Home Support Program (CHSP) provides acess to entry-level services for people who want to continue living well in their own home and community. Services can include personal care, domestic assistance, home safety and maintenance, short breaks, social activities, health and wellness and support for family and carers. People who are 65 or over (50 or over for Aboriginal or Torres Strait Islander people) and people who are 50 years or older (45 years or older for Aboriginal and Torres Strait Islander people) and on a low income, homeless, or at risk of being homeless. Home Care Packages provide a coordinated package of services for people with more complex needs who want to live well in their own home and community. Services can include personal care, domestic assistance, home safety and maintenance, short breaks, social activities, health and wellness and support for family and carers. Eligibility is based on your care needs and is determined through a free face-to-face assessment with My Aged Care. Find out more about Home Care Packages.
Veterans’ Home Care funds home care services to maintain the health, wellbeing and independence of eligible veterans and their families. Services can include domestic help, personal care, respite care and safety-related home and garden maintenance. This program is for those who need low levels of care in their own homes and who have a Veteran Gold Card or a Veteran White Card with an accepted service-related condition. If you’re not eligible for government funding or are eligible but are waiting for your government funded services to become available, you can also fund your Centacare services privately.

How to make a complaint

If you have a concern about the support or services you receive, it is important to talk about it. You have the right to make a complaint at any time.

You can contact Centacare directly to make a complaint to us by completing the form on our contact page.

You can also make a complaint about your Centacare aged care services which are subsidised by the Australian Government through the Aged Care Quality and Safety Commission.
