How to access our services

The government provides funding to support people to live independently in their own homes and community. You may be able to access our services under one of these funding schemes. Call us on 1300 236 822 to find out what you are eligible for.

Centacare is an approved provider of services under the following funding schemes:

Queensland Government 
Home Assist Secure
Home maintenance and modification services to help older Queenslanders live well in their own homes. Services include free information and referrals, in-home assessments, minor modifications such as hand rails, ramps, smoke alarms, repairs and maintenance. People over the age of 60 who are homeowners or live in rental housing are eligible for free information and referrals to reputable tradespeople and other community services. To be eligible for subsidised assistance, you must also hold a Pensioner Concession Card, be unable to complete the work yourself and be unable to make use of other forms of funding.
Extended Hours Dementia Respite
Tailored services and support for carers of people living with dementia and neurodegenerative conditions. Services can include social outings in the community, centre-based activities and readying and settling (in-home support).    People who are working or looking for work, studying or volunteering while caring for a person who has dementia or a neurodegenerative condition. Carers do not have to live with the person they are caring for to be eligible.
Australian Government
Commonwealth Home Support Program (CHSP) 
Entry-level services for people who want to continue living well in their own home and community. Services can include personal care, domestic assistance, home safety and maintenance, short breaks, social activities, health and wellness and support for family and carers.   People who are 65 or over (50 or over for Aboriginal or Torres Strait Islander people) and people who are 50 years or older (45 years or older for Aboriginal and Torres Strait Islander people) and on a low income, homeless, or at risk of being homeless. 
Home Care Packages 
A coordinated package of services designed for people with more complex needs who want to live well in their own home and community. Services can include personal care, domestic assistance, home safety and maintenance, short breaks, social activities, health and wellness and support for family and carers.   Eligibility is based on your care needs as determined through a face-to-face assessment.
