Description:

Centacare is the social services arm of the Catholic Archdiocese of Brisbane and has been operating in South East Queensland for over 50 years.

Centacare Community Services (CCS) directorate is structured into 6 regions with a centralised support service delivering aged care, mental health, housing support, homelessness, respite and transition, independent living and support for people with a disability and learning and leisure services. 

 

The job

Centacare is seeking 2 full time Service Delivery Managers (SDM) in the Fraser Coast area to operate from Hervey Bay. 

The SDM will ensure the service delivers individual, flexible, innovative, responsive and timely support to service users in conjunction with Centacare's mission vision and values. This is delivered by contributing to a positive workplace culture with a primary focus on client experience, brand awareness and commercial focus in an adaptive and changing environment.

In this role you will:

• Ensure Client Support Plans are developed, implemented, regularly monitored and data accurately recorded;
• Ensure contracted funding outputs are met and supports delivered as per service agreements;
• Engage clients in all aspects of their service delivery and negotiate change/options agreeable and beneficial to the client in line with legislative and funding body requirements;
• Employ specified positive behaviour strategies with clients who may be exhibiting challenging behaviour;
• Manage a workforce which is diverse in age, culture and geographical location;
• Respond and adapt to changing environments;
• Actively engage in change management; and
• Be accountable for Client engagement and retention.

What we need from you

• Diploma in Community Services, Social or Business Management OR several years experience in Coordination or Case Management or other relevant industry experience;
• Strong interpersonal and communication skills and the ability to liaise effectively with others;
• Ability to prioritise workloads to meet schedules;
• Strong decision-making, conflict resolution and problem solving skills;
• Proficiency in IT standard software such as Microsoft Office suite and Internet;
• Proven financial literacy skills;
• A current QLD Drivers Licence;
• A roadworthy, registered and suitably insured vehicle to use for work purposes;
• Commitment to person centred and self-directed support;
• Strong leadership qualities;
• Physical ability to carry out all tasks required of the position;
• The ability to obtain a Positive Notice Working with Children "Blue" Card and a Disability Services Exemption "Yellow" card; and
• The ability to obtain a National Police Certificate.

Remuneration

You will be paid in accordance with our Collective Agreement with the base salary commencing at $74,243 pa ($37.57 /hr) or higher depending on qualifications and/or previous experience.
