Individuals receiving funding

For individuals who want greater control over their funding and support needs.

Centacare Choices provides support to individuals who are currently in receipt of funding from the Department of Communities, Child Safety and Disability Services and wish to self-manage their funding and make choices about how their funding is spent.

Individuals not currently receiving funding

Specialist Disability Support

Individuals who wish to access a Centacare Specialist Disability Service, but do not currently receive a funding package, needed to first be assessed by the Department of Communities, Child Safety and Disability Services.

Other support available

Centacare also provides support through the Queensland Community Care Service. This support is available for individuals who are:

  • aged 65 and under (under 50 for Aboriginal and Torres Strait Islander people)
  • having difficulty performing core activities of daily living; and
  • at risk of losing their independence or having to go into long term residential care,
    or
  • a carer of an individual eligible to receive a Queensland Community Care Service.
