Home Care Packages

If you are finding that you cannot manage some activities around the home, that you used to be able to do, a home care package could help. Through assistance tailored with understanding and sensitivity to your individual care needs, a home care package provides help with a range of activities in and outside of the home. This could include cleaning and gardening, preparing meals or transport to buy the weekly groceries.

A home care package helps you to stay in your home longer and keep you doing the things that are important to you.

Home Care Package Services

Centacare offers a range of services that people can access through their home care package. This could include:

  • personal care – bathing or showering, dietary requirements – including the delivery and preparation of meals, dressing, fitting sensory aids, safety and emergency assistance aids and help getting around the home
  • support services – domestic duties such as help with cleaning and dusting the house, washing and ironing, pet care, home maintenance and modifications related to your care needs, respite care and transport to go shopping, visiting your doctor or taking part in social activities
  • clinical care – nursing, allied health and other therapies such as physiotherapy, wound care, continence aids, dietitian support and mobility aids and equipment

Your Centacare Case Manager will work with you to help determine the type of services that best suit your needs, based on the goals that you want to achieve.

A key element of any home care package is the services that are provided. That’s why at Centacare we offer quality services that are administered by professionally trained staff. At the same time we believe providing emotional support and companionship is just as important.

 Getting started

There are government eligibility criteria for Home Care Packages. You will need to contact My Aged Care and they will organise a free assessment with a member of an Aged Care Assessment Team (ACAT).

During this process, indicating that you would like Centacare to look after your home care package needs, will ensure you have access to the benefits of a Centacare Home Care Package.

There are four levels of Home Care Packages available, with the level of care determined during the ACAT assessment and based on the amount of care each person requires.

  • Level 1 packages supports older people with basic care needs
  • Level 2 packages supports older people with low-level care needs
  • Level 3 supports older people with intermediate care needs
  • Level 4 supports older people with high-level care needs
